This program is effective May 1,
2004 and revised June 1, 2006.

If the above criteria is met,
the employee will receive a monthly safety point valued at approximately
$10.00, which can be used to purchase various safety award items.
3.
The safety points will be
recorded on the employee’s check stub and a sticker will be mailed with the
check stub. The check stub will show total points available and adjustments will
be made when the rewards are redeemed.
5.
To redeem points, employees go
to Kokosing’s website
www.kokosing.biz and select “Safety Recognition.” Items available, order
forms, criteria for this program and instructions will be posted. The form
can also be requested from your superintendent.

Items may be changed or
discontinued based on availability.
8.
The certificates from the
“Safety Saves” program cannot be used in this Safety Recognition program, and
must be redeemed using the Safety Saves forms prior to 12/31/04.
9.
Any employees not reporting an
accident, injury or falsifying a safety talk or other records are subject to
disciplinary action up to and including termination.
